Full Array of Home and Community-Based Services Ohio

For nearly two decades, we have met the needs of our clients by tirelessly providing exceptional services every step of the way. We serve the elderly, physically disabled, and/or intellectually or developmentally disabled adults through programs tailored to the individual and his or her situation and supports. Services can be provided in a residential or family based setting, in the community, and/or the individual’s personal home.

Community Living

We support individuals in home and community settings to promote independence, self-determination, and community involvement. Services may include delegated nursing, waiver nursing, remote support, respite, and community transition. Employment supports such as Career Planning and Individual Employment Support are included to help individuals find, secure, and thrive in meaningful community employment.

adult day service

Structured day supports focused on skill development, social engagement, and employment readiness. Additional supports may include vocational habilitation, community transition, remote support, and other goal-based services.

healthcare service

Coordination of health-related needs including medication training and oversight, medical scheduling, and health advocacy. Nursing supports include delegated nursing and waiver nursing to ensure safe and consistent care.

Bryan Smith

Client Relations Director – Ohio

Bryan loves helping others as evident with over 25 years of experience in the pharmaceutical and hospice industries. He is passionate about supporting individuals with intellectual and developmental disabilities to help them reach their full potential and independence.

Kyrra McIntosh

Client Relations Director – Ohio

Kyrra has had a dynamic and impactful career with Caregiver Inc., beginning in 2021 as Area Director of Operations. In this role, she improved operational efficiency and led diverse teams, enhancing care facility management and client outcomes. After a successful tenure, she broadened her expertise in Admissions and Marketing, driving census growth and creating waitlists for rooms in three facilities. Kyrra returned to Caregiver Inc. as Client Relations Director, where she combines her operational leadership and marketing experience to strengthen client relationships and ensure both clients and staff thrive. Her strategic mindset and interpersonal skills make her an invaluable asset to the company.

Bio

Kyrra lives on an 80 acre farm in Ohio where she and her husband raise show pigs.

Outside of work, Kyrra enjoys spending time at the beach with friends and family.

With experience in client relations and a background in business development, Bailey works closely with families in the IDD space to ensure they receive the best possible support. Dedicated to fostering meaningful connections, Bailey is committed to making a positive impact in the community.
