Southern Concept’s homes offer a warm family-like environment while promoting independence. Southern Concepts day habilitation programs strive to provide each individual with skill development, socialization and a meaningful day. The individual’s planning team works diligently to determine daily programming desires, whether that be community employment, volunteer activities or life skill training through our day habilitation center.

Our History

Southern Concepts was founded in 1980 with one group home located in Granbury, Texas. Now, as part of the Caregiver, Inc. family of care services, we serve hundreds of individuals through the ICF-IDD, HCS, and Texas Home Living (TxHml) programs.

Our Dayhab Centers

SCI Madison Park Dayhab

Located in Granbury TX, consumers participate in activities, attend local events, and work in our “store” that keeps the group homes stocked with supplies. This location also offers volunteer roles with local Meals on Wheels and other opportunities. Participants at this program location are in groups according to level and ability and are able to share responsibilities. Staff work with individuals to establish and track measurable goals.

SCI Boston Dayhab

Located in Benbrook, TX, consumers participate within designated labs according to their age and level of participation. Each individual is able to complete daily activities, enjoy walks, and participate in community activities as they are able. Staff work with individuals to establish and track measurable goals

Services

Our focus at Southern Concepts (SCI) is to ensure that our consumers are able to live their lives to the fullest while having the highest level of care possible. We want our consumers to feel at home, while also learning the responsibilities that are required to be a productive member of their household.

  • Our staff are 24 -hour staff with consistency in weekly scheduling.
  • Rotating menus are approved by a dietician to offer a balance of variety and nutrition.
  • Residents of our homes participate in housekeeping and cooking duties as able, allowing them to learn to be a productive member of their household.
HCS Program
The Home- and Community-Based Services (HCS) program promotes and maintains consumer and family involvement in providing individualized program development and community support services for individuals with an intellectual or developmental disability. The HCS program is a Medicaid waiver program and provides services and supports to eligible individuals as an alternative to Intermediate Care Facilities for individuals with intellectual disabilities (ICF/ID) program.

ICF/IID Service
Intermediate Care Facility for Individuals with Intellectual Disabilities (ICF/IID), is a Medicaid-certified program, serving 6-13 individuals in a residential group home setting with 24-hour shift staff. Individuals are evaluated, and an individualized treatment plan is developed to assist each individual function at their greatest ability.

Home and Community Based Services (HCS)

Home and Community Based Services (HCS)

The Home- and Community-Based Services (HCS) program promotes and maintains consumer and family involvement in providing individualized program development and community support services for individuals with an intellectual or developmental disability. The HCS program is a Medicaid waiver program and provides services and supports to eligible individuals as an alternative to Intermediate Care Facilities for individuals with intellectual disabilities (ICF/ID) program.

How to obtain this service?

Enrollment & Eligibility

The individual must have a determination of mental retardation or a related condition, Medicaid and SSI eligibility, and their Plan of Care must meet program guidelines. HCS slots are filled from a waiting list by the community Mental Retardation Authority (MRA). Individuals who are currently enrolled in an HCS program can also transfer to another service provider.

  • Your Local Intellectual and Developmental Disability Authority (LIDDA) will assist you in determining eligibility and completing the paperwork so that you may receive HCS services.
  • When there is a wait list for HCS services, stay in touch with LIDDA to be contacted when an opening becomes available.
  • LIDDA will provide a list of service providers from which you can choose to visit. If you’d like to visit us, contact us to schedule a tour.
  • If you are currently receiving HCS services from any service provider and are unsatisfied with the services, contact your local LIDDA to discuss transfer options.

HCS Service Options

Individualized Care Plans

Each consumer possesses a unique and individualized Individual Service Plan (ISP) and Individual Plan of Care (IPC) based on the participant’s skills, strengths, needs, and desires. The Individual Service Plan and the Individual Plan of Care are designed to meet and provide for the unique and personal needs of each person enrolled in the HCS program, with an emphasis on outcomes and supports needed to increase overall independent functioning of the individual. The HCS program offers the participant a full array of supportive and therapeutic services. The HCS program also promotes community involvement through the use of community supports and services for medical, social or day program needs, depending on the specific needs or choices of the individual.

Residential Services Options

This residential option provides services and supports as needed by individuals who choose to or who would benefit from living in a three or four person home. A three person home provides for supervised living where shift staff and/or live-in staff provide assistance with daily living activities to acquire skills necessary for independence. The staff are present in the residence and are able to respond to the needs of individuals on a 24-hour basis. A four person home provides for supervised living where shift staff are present in the residence and awake whenever and individual is present in the home. The staff provides assistance with daily living activities and training in self-help and independent living skills.

This residential service option, formerly known as “Foster Care” can be an excellent choice for many people in the HCS program.  As the title indicates, the individual lives with the host home provider in his or her home.  The provider assists the individual with daily activities and helps to establish and meet the individual’s personal goals.  Additionally, the provider facilitates transportation to day habilitation or a job in the community as well as inclusion in community activities.

Biological parents can provide this service to their adult children in the HCS program.  Southern Concepts can also help individuals find a host home provider that is compatible and has the ability to meet the needs of the individual.  Southern Concepts provides all necessary training and strives to support the provider with administrative tasks as much as possible to keep the focus on the individual.

PAS/HAB: This support service option is supported by staff who coordinate and provide services to an individual living in his or her own home or the home of his or her natural or adoptive family members, or to an individual receiving host home/companion care services from Department of Family Protective Services. The individual is provided direct personal assistance with activities of daily living and coordination of community activities and skills development toward greater independence.

Case Coordination: The coordination, integration, evaluation, monitoring and service delivery of all services provided to the individual.

Nursing Services: Nursing services are provided to ensure the general health and well being of the individual. Nursing services provided by LVNs, RNs and 24-hour On-Call nursing staff.

Respite Services: Temporary, short-term and/or emergency care provided as a break for the primary caregiver. Care provided in the individual’s home or at another location.

Day Habilitation: Day activities designed to assist individuals in acquiring, maintaining and improving self-help, socialization and adaptive skills necessary to reside successfully in the community.

Psychology Services: Services are designed to develop, modify or improve the cognitive, social and affective skills of the individual through the use of behavior intervention and learning theories.

Dental Treatment: Services as determined by individual needs including emergency, preventative, therapeutic and orthodontic treatment.

Specialized Therapies: Audiology services, Speech/Language pathology services, Occupational Therapy services, Physical Therapy services and Dietary services to support the needs of the individual.

Adaptive Aids: Adaptive devices, controls or appliances, which enable persons to maintain or increase their abilities to perform activities of daily living or control the environment in which they live.

Minor Home Modifications: This component includes assessing the need, arranging for and providing modifications and/or improvements to an individual’s home to allow for community living while ensuring safety, security and accessibility.

Supported Employment: Services provided in an integrated setting to include ongoing support, supervision and training needed to sustain paid work for the individual.

Intermediate Care Facility (ICF/IID)

Intermediate Care Facility for Individuals with Intellectual Disabilities (ICF/IID), is a Medicaid-certified program, serving individuals in a residential group home setting with 24-hour shift staff. Individuals are evaluated, and an individualized treatment plan is developed to assist each individual function at their greatest ability.

ICF/IID is different from HCS services, in that, an individual who has not yet received his/her HCS slot from the local Mental Retardation Authority (MRA), as long as he/she has been diagnosed as having intellectual disabilities, can receive residential services, assuming there are vacant ICF beds available.

Enrollment Information

There is not a waiting list for ICF/IID services and immediate placement may be available.

Eligiblity

The individual must have a diagnosis of mental retardation, be in need of and able to benefit from the active treatment provided in the 24-hour supervised setting and meet the Medicaid criteria and the Level of Care determination criteria.

Enrollment Procedure

Each individual receives a comprehensive functional assessment to determine strengths and needs. Individuals choose personal goals, which training is developed to meet.
