Position Summary
 
The Health Service Coordinator provides support to consumers in a supported living and licensed setting with competent, quality oversight and coordination of their health care needs. We provide only supplemental coordination of health care needs of the consumers and quality assurance reviews in compliance with OAC 5123:2-6-07.

  • Completion of Quality Assurance Reviews on a Monthly, Quarterly, and Annual basis
  • Admission of new consumers by assessing their self-medication needs, completion of medical history composite, health service plan and review and update of new medication orders
  • Provide Nursing Delegation when needed and Individual Specific Training for Direct Service Personnel
  • Review, update and make recommendations for consumer appointments using Every Healthy Person as a guide on an ongoing basis
  • Provide on-going communication of health-related concerns and updates to the Residential Provider and become a collaborative part of the team and coordinate care of the consumers with internal and external team members.
  • Count and monitor the use of control substances in accordance with OBN standards and review that staff are documenting correctly and report any discrepancies to the Residential Provider immediately
  • Review all appointment visit summaries, hospital discharge, emergency room visits, to ensure all changes have been fully and accurately documented within 24 hours
  • Completion of monthly changeover of medical records and review of all medications delivered from the servicing pharmacy
  • Assist with dietary and health recommendations such as what to look for on food labels and dietary needs
  • Maintain open communication with Residential provider, guardians, treating physicians, specialist, pharmacy, county board professionals and other supports
  • Report abnormal findings to physician and Residential Provider in accordance with standards of the Ohio Board of Nursing, laws and rules as well as agency policies and procedures
  • Provide educational training to enhance the staff’s abilities regarding the health needs of the consumer served
  • Attend ISP and BSP meeting per invite and as schedule allows
  • Travel to consumers home sites at various times 1-2 times per month and for special situations that may increase visit times with travel expected at least 50% of the time
  • Modify Health Service Plan when warranted by changes in consumer condition, treatment or service needs.
  • Provide 24 hour a day, 7 days a week on-call consultation for the consumers, his/her staff, residential provider and county board
  • Use of electronic medical record for billing purposes while maximizing productivity
  • Fulfill additional roles and performance of other duties as assigned based on consumer and residential provider needs as assigned by Supervisor or upper management
  • Other duties as assigned.
 
 
 
Qualifications
 
Education and/or Experience
 
  • Graduate of an accredited School of Nursing
  • Active license in the State of Ohio
  • Two years nursing experience with experience in the IDD field
  • Certified in CPR through an approved provider
  • Demonstrated competence through practical and written test
 
Knowledge and Skills
 
  • Shows leadership ability, using thoughtful approach to staff needing direction or making request while giving clear direction
  • Able to organize time and activities so as to complete assignments timely, effectively and accurately.
  • Utilizes the Nursing Process in developing Health Service Plans for all HSC consumers
  • Able to respond to stressful situations both with patients, families as well as with co-workers in a constructive, timely, and professional manner
