Welcome to Daybreak
Daybreak has been in business for almost 40 years. Daybreak offers a full array of home and community-based services (HCS) including residential group homes, host home/companion care, ICF homes, and supported home living. Daybreak also operates several day habilitation sites around the state.
Daybreak was founded on a commitment to provide innovative supports and community-based services to people with intellectual and developmental disabilities. This commitment guides us each day. We provide a supportive environment in which each individual may make and achieve goals while experiencing community life at its fullest.
Our roots in the community go back to 1978 with the opening of the Lancaster Residential Center, an intermediate care facility for individuals with developmental disabilities (ICF/ID). In the late 80’s and 90’s, seven additional ICF/ID homes were opened in Dallas County. Lancaster Residential Center voluntarily downsized in 2010 to enable the individuals there to live in smaller, group home settings.
Daybreak Community Services, Inc. was founded in 1997 to provide an array of Home and Community-based (HCS Medicaid-funded waiver) services to individuals with intellectual and developmental disabilities. Daybreak has continued to expand services throughout Texas during the last two decades.
Now, as part of the Caregiver, Inc. family of care services, Daybreak operates HCS waiver homes and ICF/ID group homes, as well as services in individual homes, host home settings, and day habilitation sites.
The Daybreak management team has extensive experience in developing and managing programs for people with disabilities. Local staff, with deep community connections, deliver personalized care at each of our locations.

Home and Community Based Services (HCS)
The home- and community-based services (HCS) program promotes and maintains consumer and family involvement in providing individualized program development and community support services for individuals with an intellectual disability. The HCS program is a Medicaid waiver program and provides services and supports to eligible individuals as an alternative to intermediate care facilities (ICF/ID) program.
An individual must have a determination of intellectual disability or a related condition, Medicaid and SSI eligibility, and their Plan of Care must meet program guidelines.

Residential Service Options
This residential service option, formerly known as “Foster Care” can be an excellent choice for many people in the HCS program. As the title indicates, the individual lives with the host home provider in his or her home. The provider assists the individual with daily activities and helps to establish and meet the individual’s personal goals. Additionally, the provider facilitates transportation to day habilitation or a job in the community as well as inclusion in community activities.
Biological parents can provide this service to their adult children in the HCS program. Daybreak can also help individuals find a host home provider that is compatible and has the ability to meet the needs of the individual. Daybreak provides all necessary training and strives to support the provider with administrative tasks as much as possible to keep the focus on the individual.
